Dr Caldwell Esselstyn (b 1933) is a former heart surgeon from Cleveland Clinic. As of 2020, he's 86 years old and still active. His family regularly makes YouTube videos showcasing WFPB lifestyle and cooking techniques — check out Jane Esselstyn's YouTube Channel (Jane is Dr Esselstyn's daughter).

Personally, I find Dr Esselstyn's mind-boggling, considering that Heart disease is the #1 killer of men and women worldwide and heart disease rates are exceptionally high among Indians.

20-Year Study

Dr Esselstyn conducted a 20-year study over a small group of 18 patients. The text below has been copied from his website:

The patients in Dr. Esselstyn's initial study came to him with advanced coronary artery disease. Despite the aggressive treatment they received, among them bypasses and angioplasties, 5 of the original group were told by their cardiologists they had less than a year to live. Within months on Dr. Esselstyn's program, their cholesterol levels, angina symptoms, and blood flow improved dramatically. Twelve years later, 17 compliant patients had no further cardiac events. Adherent patients survived beyond twenty years free of symptoms.

Drop in cholesterol levels: After 5 years on Dr. Esselstyn’s plant-based diet, the average total cholesterol levels of his research group dropped from 246 milligrams per deciliter to 137 mg/dL (Above 240 mg/dL is considered “high risk,” below 150 mg/dL is the total cholesterol level seen in cultures where heart disease is essentially nonexistent.) This is the most profound drop in cholesterol ever documented in the medical literature in a study of this type.

Cardiac events: The 17 patients in the study had 49 cardiac events in the years leading up to the study, and had undergone aggressive treatment procedures. Several had multiple bypass operations. After beginning the eating plan, there were no more cardiac events in the group within a 12-year period.

Angiogram evidence: Angiograms taken of the participants in the study show a widening of the coronary arteries, and thus a reversal of the disease

Comparison with Ornish Reversal Program: Two broad differences between Dean Ornish's program and Esselstyn's guidelines relate to dairy and stress reduction. Esselstyn does not allow any dairy and he does not emphasize stress reduction, making it optional. According to him, simply changing food habits and ensuring daily exercise is sufficient for reversing heart disease. Both Ornish and Esselstyn emphasizes complete elimination of oils for heart patients.

Dr Esselstyn's Diet: Summary of Results

A very readable, short paper by Dr Esselstyn summarizes his heart health studies:

(1) Dr Esselstyn's first study with 18 patients: An excerpt from the research paper above:

In 1985, I initiated a small study utilizing WFPBN in 24 patients who were severely ill with CAD. The moment of truth came a year later for a patient in his 50s with significant vascular disease in his heart and right leg. At study onset, while crossing a skyway into the Cleveland Clinic he experienced claudication that forced him to stop and rest five times. Results of a pulse volume study revealed a markedly diminished right ankle pulse. Following nine months of WFPBN, all claudication had resolved, and a repeat pulse volume was double that of his baseline.[11] As statin drugs were not yet available, this was proof of the concept that WFPBN alone can halt and reverse CVD.

After 12 years, we reviewed the CVD events of our 18 adherent patients. During the eight years prior to entering our study, while in the care of expert cardiologists, they had sustained 49 cardiac events, which was indicative of disease progression. In contrast, 17 of the 18 patients sustained no further events during the 12 years they spent in our study. One patient who was initially adherent developed angina and required bypass surgery six years after resuming a Western diet. These findings illustrate the need for close adherence with WFPBN.[12]

Twelve patients in our initial group had a follow-up angiogram. There was significant disease reversal in four of them, which suggests that WFPBN could not only eliminate future cardiac events but could also reverse angiographic disease. Even if significant angiographic reversal does not occur, patients can still benefit from more subtle improvements in the overall health of their endothelial cells and their capacity to produce nitric oxide, eliminating angina and future CVD events.[12] Additionally, the robust antioxidant value of WFPBN diminishes oxidative inflammation in plaques and foam cells, thus strengthening the cap over the plaques (Figure 1).

(2) Dr Esselstyn's second study with 198 patients: An excerpt from the research paper above:

In 2014, we conducted a second larger study of 198 patients with significant CAD.[9] Of these patients, 119 had undergone a prior coronary intervention with stents or bypass surgery, and 44 had a previous heart attack. There were multiple comorbidities including hypercholesterolemia, hypertension, obesity, and diabetes. During four years of follow up, 99.4% of the participants who adhered to WFPBN avoided any major cardiac event including heart attack, stroke, and death, and angina improved or resolved in 93%. Of the 21 non-adherent participants, 13 (62%) experienced an adverse event. When comparing these results to the well-known COURAGE,[13] and Lyon Diet Heart Study,[14] which consisted of conventionally treated participants, there is beyond a 30-fold difference in major cardiovascular events favoring WFPBN.

    This article was written at the 12-year mark of the 20-year longitudinal study that tracked 18 patients who were initially very sick. The results at the end of 20 years were reported in Dr Esselstyn's book 'Heart Attack Proof (see above).

Personal Narrative

Personal Narrative: Olympic Reflections 50 Years Later, published in Yale University Alumni Magazine. Dr Esselstyn recounts the final race in 1956 Olympics — he was part of the rowing team that won the gold medal. In the latter half of the article, Dr Esselstyn ruminates on the state of cardiology, heart disease and food.
